Two Endicott faculty recommendation forms: If your program provider also requires faculty reference forms, the Office of Undergraduate Programs will accept those in lieu of Endicott forms.

Copy of your passport ID page: Student MUST apply for passport if they do not have one or renew an expired passport immediately. It is recommended that your passport is valid up to six months past the completion of your program.
Request for Course Approval form: Using the course list for your intended program, please complete the Pre-Registration Course Approval form with your Advisor or Academic School Dean.
